ASSUMPTION OF RISK
I understand that dance is a physical activity and that participation involves risk of injury, including but not limited to sprains, strains, fractures, and other injuries. I understand these risks exist even when reasonable care is taken. I voluntarily enroll my child with full knowledge of these risks and accept them.

RELEASE OF LIABILITY
In exchange for my child being permitted to participate in classes, performances, and activities at Natalie's Studio of Dance, I release Natalie's Studio of Dance, its owner, instructors, employees, and volunteers from claims arising out of my child's participation, except for claims arising from gross negligence or willful misconduct.
I understand that NSOD does not provide medical or accident insurance for students. I am responsible for maintaining adequate accident and medical insurance for my child.

MEDICAL TREATMENT AUTHORIZATION

If my child is injured or becomes ill and I cannot be reached, I authorize NSOD staff to seek emergency medical treatment for my child. I accept financial responsibility for any treatment provided.

STUDIO POLICIES

I have read and agree to the attendance policy and the tuition and payment terms, including the monthly payment schedule and the processing fee added to each payment.
